You can use this sample credit report dispute letter to ask the credit bureau to delete or correct any inaccurate information on your credit report.
Enclose a copy of your credit report with the disputed items encircled. Include any documents (Copies – NOT originals) that support your position.
Send your credit dispute letter via certified mail, return receipt requested, so that you have document that the credit bureau received your correspondence.

Dear Sir or Madam:
I’ve recently obtained a copy of my credit report from your agency, and found the following items to be in error (these items which I dispute are also encircled on the attached copy of my credit report):
Item 1: I dispute [Name of source such as creditor/tax court etc.] account number [identify type of item, such as credit account, judgment, etc]. This item is [inaccurate/incomplete] because [describe what is inaccurate or incomplete and why]. I am requesting that the item be [updated/deleted/other specific change] to correct the information.
Enclosed are copies of [use this statements if applicable and describe any enclosed documentation, such as payment records, bank records, checks, court documents etc.] to support my position.
Item 2: I dispute XYZ Credit Card Company account number 12345. This item is incorrect because the account has been paid in full. I am requesting that the item be removed.
Enclosed are copies of the XYZ Credit Card that support my position.
I am requesting that you conduct an investigation of these items pursuant to Section 1681 of the Fair Credit Reporting Act.
I request that you investigation include, at the very least, that you request verification of each item from the furnishers of information, and that you attempt to verify the accuracy of the alleged verification. Finally, I request that you make any deletions and/or corrections to my credit report that are warranted as a result of your investigation. Specifically, any items that cannot be verified by the furnisher of information should be promptly deleted. Further, any negative items that are considered “outdated” or “incomplete” under the Fair Credit Reporting Act should likewise be deleted from my credit file.
Finally, I’m reminding you that you are required to conduct the subject investigation in a timely manner. The Fair Credit Reporting Act states that the investigation should be completed within 30 days.
Please send me a copy of my updated credit report to the address listed above following the completion of your investigation.
Please also provide me with any documentation that you received in connection with the above investigation including, but not limited to, the telephone and/or contact information of the furnisher[s] of information, any and all responses provided by the furnishers of information, and the specific action taken as a result of those responses.
